Creating roadmaps to recovery, Collage Home and Community brings intensive neurorehabilitation to the most functional setting there is—a patient’s home and community.
Our comprehensive rehabilitation teams develop integrated treatment plans to bring about change. We address patient centered goals through intentional activities that match our patients’ interest and needs. After completing the program, our outcomes show that patients and families have stronger abilities, coping and adjusting skills, and greater participation in life – which is a powerful predictor for quality of life after a brain injury.

Company History
In July of 2020, ReMed and Learning Services came together to form Collage Rehabilitation Partners. Collage continued to expand its continuum of services by acquiring Tree of Life in Virginia and the former Restore program in Georgia and rebranding that program as Learning Services. Collage Home & Community Rehabilitation Partners expanded from the original two programs in Texas and California through acquisition in Colorado and organic growth in Georgia and South Carolina.
